I’ve used google forms and docs extensively to build free questionnaires and spreadsheets to collect and display data I need to help monitor the wellbeing of my athletes. I’ve used this data to help build excel databases, such as the one I have made available here, as well as help build bespoke databases for other coaches.sports scientists. One of my most popular posts has been a step by step guide on how to build these databases and questionnaires for free
I’ve decided to show you what a questionnaire can look like and give you the ability to fill in the questionnaire and view how it looks on a google doc. You can see how easy it is to structure something like this as well as copy some of the formulas I’ve used to display the data how I have here.
Workload Questionnaire & Dashboard
Click here to be taken to the workload questionnaire, where you can input rpe x duration as well as session type, time and add custom comments. You can view (and only view) the data you have submitted on the dashboard here.
Wellness Questionnaire & Dashboard
Click here to be taken to the wellness questionnaire, where you can input ratings for sleep, fatigue, soreness, stress, illness or injuries. You can view (and only view) the data you have submitted on the dashboard here.
Hopefully this gives you some idea of the potential of these free tools, use the questionnaire’s on your phone, laptop and tablet to see how they may look to your athletes. As I’ve gone through in this post it is so easy to customize these questionnaires and dashboards. And once you download the data to excel, it gets a whole lot more fun!
